how to apply 2 uvw map channels to a mesh in unity ?

It depends on your 3D app. In Blender, click on the UV Texture "New" button. (You can add any number, but Unity only uses the first two.)

where in unity do i found these settings for the channels, we use 3ds max for export

If you're talking about scripting, it's $$anonymous$$esh.uv and $$anonymous$$esh.uv2. If you're talking about shaders, it's texcoord0 and texcoord1.
